We implement advanced encryption and secure data protocols to protect information processed by the AI system, ensuring it is safely integrated into the physician’s EMR.

HIPAA Authorization Form
To provide more accessible healthcare services, ViClinic now offers medical consultations via telehealth, including the use of AI-driven technology. This allows healthcare providers to collect, summarize, and evaluate your symptoms before your appointment.

By signing this HIPAA Authorization Form, you consent to the collection, use, and disclosure of your personal health information as follows:

AI-Driven Pre-Visit Symptom Deep Dive: Your health information (symptom descriptions, intensity, and other details) will be collected by the AI system before your visit.AI-generated summaries will be shared with your healthcare provider and integrated into the electronic medical record (EMR) for efficient evaluation.

Disclosure and Use: Your health information may be used for diagnosis, treatment, and medical decision-making during telehealth consultations.The collected data will be stored securely and shared with your healthcare provider only as needed to facilitate care.

Security and Privacy Protections: Your health information will be handled in accordance with HIPAA regulations, including secure transmission and storage of AI-collected data.You retain the right to revoke this authorization at any time, with the understanding that certain information may be necessary to continue your care.

Informed Consent for Telehealth Visit
By agreeing to use ViClinic’s telehealth services, you are also consenting to the integration of ViClinic’s AI-driven Pre-Visit Symptom Deep Dive feature in your care. This feature collects comprehensive health information prior to your appointment, allowing your provider to focus on more personalized diagnosis and treatment.

Risks Associated with AI-Driven Data Collection:
- The AI system may not capture all relevant details, necessitating additional questions from your provider during the consultation.
- Technical issues may arise during data transmission, which could result in delays.

By signing below, you acknowledge:
- You understand the nature and scope of ViClinic’s AI-driven Pre-Visit Symptom Deep Dive and its role in telehealth services.
- You are aware of the potential risks, including data accuracy concerns and technical delays.
- You consent to the collection and use of your health information through AI systems as part of your telehealth care.
